Endotypes of angina with non-obstructive coronary arteries: a prospective multicentre study (European Heart Journal)

The study investigates the underlying mechanisms of angina pectoris without relevant coronary stenosis (ANOCA – Angina with Non-Obstructive Coronary Arteries), a common form of myocardial ischaemia that particularly affects women. Standardised coronary function tests identified eight different haemodynamic endotypes in over 1,000 patients, including epicardial and microvascular spasms, increased coronary resistance and endothelial dysfunction. Each endotype was associated with specific clinical characteristics. Endotype-specific drug therapy recommendations were defined on the basis of a structured Delphi consensus process. The results enable mechanism-based stratification of ANOCA and lay the foundation for more precise, individualised treatment.
